Contacting our office, or submitting any of the information below, does NOT ensure that we will provide representation to you. You should not assume that any deadline you are facing will be stayed, tolled, or in any way delayed by the fact that you have contacted our office or sent any materials to us. We will not take any action on your behalf unless or until we have offered and received a written Representation Agreement signed by you.

Although we do not represent you unless we offer and you accept representation, and then a Fee Agreement is executed, any information you provide to us on the form is still privileged, because you are seeking legal representation. It is important, however, that you not discuss any communications you have had with us, or you may waive the privilege of confidentiality.

Our sending you this form, and/or our review of the information once it has been returned to us, does not mean we represent you. Therefore, if you have any deadlines or requirements to meet prior to the time we may reach an agreement of representation, you should be certain to take whatever steps you may need to take without regard to whether you may be represented by us at some future time. If you have been told you have a deadline by which to act, or if you have received a Notice of Right to Sue from the Equal Employment Opportunity Commission or local human rights agency, please notify our office immediately of these deadlines. If we determine we cannot meet with you with sufficient time to meet those deadlines, we will provide you with names and numbers of other attorneys for you to contact who may be able to meet those deadlines.

Because we have a small office, we screen our cases and take only a select number, based upon a number of factors, to ensure we are able to provide quality services to our clients. In the event we do not offer you representation, we will provide you with names of other attorneys with whom we are familiar and believe may be able to offer you representation. In the event you have a matter that we do not handle, please feel free to contact our office, as we may be able to refer you to attorneys with whom we are familiar who practice in that area.

We may also ask you to prepare a chronology and gather documents to assist in our intake process.
